PH Connect is an online tool designed to support Australian healthcare professionals (HCPs) in referring suspected Pulmonary Arterial Hypertension (PAH) patients to PAH expert centres for diagnosis and treatment.
Patients with unexplained dyspnoea (breathlessness), for which common causes have been excluded, may be at risk of having PAH and should have their symptoms investigated until a cause is found. This tool can support your case for referral of such patients by directing you to appropriate diagnostic tests for raising the suspicion of PAH, and then connecting you with a PAH expert centre.
Complete a short step-by-step questionnaire to receive immediate guidance on raising a PAH suspicion and generating a complete referral.
Please click on the button below to find the most appropriate PAH expert centre for your patients via the PAH expert clinic finder.
This is not a diagnostic tool and is only intended to support the referral process for suspected PAH patients. It does not replace expert clinical judgement or necessary procedures.
The questionnaire and recommendations have been based on approved ESC/ERS Guidelines and developed with input from Australian PAH experts.
PH Connect is only recommended to be used by physicians familiar with diagnostic methods such as transthoracic echocardiography, electrocardiography, chest X-ray and pulmonary function testing.
